PRISM™

The PRISM™ system is a unique digital toolbox with a suite of applications that measure the impact of a disease condition on the functioning and well-being of individuals. It forms the backbone of a care management system. The clinical data gathered by the PRISM™ System is used to assess disease risk and track outcomes through the course of treatment. This provides physicians the means to harness the power of their practice and to better understand how pain affects their patients.

The Patient Assessment Matrix (PAM) is an intuitive disease severity scoring system that allows physicians to compare broad-based characteristics common to patients with pain caused by a wide variety of conditions. It combines medical and psychosocial elements to generate a risk-adjusted view of a patient population. This can provide an overall assessment of the complexity of each patient’s condition, and, when aggregated, it allows for fair comparisons between different practices. It is usually assigned at the time of the initial visit or early in the course of care.
The Pain Health Assessment (PHA) is a patient-reported, multidimensional view of how pain and general health affect the individual. It can be used as a clinical outcomes instrument to assess the condition and track clinical effectiveness over the course of treatment. The assessment is designed to capture data from both new and existing patients. Thus, all patients can benefit from it, and a physician can use it for an entire practice.

The assessment is filled out by patients in the clinic; this takes between 15-20 minutes to complete, depending upon the patient's literacy level and comfort with technology. It makes use of down time in the waiting room or exam room and helps patients frame their individual pain experience into a common language.

The PRISM™ Summary Page is a dashboard that organizes and summarizes the results of the Pain Health Assessment into functional and psychosocial indices as well as other key performance groups. It displays current and trend information in real time to assist with data–informed clinical decision making. It can be used to guide quality care. Because of our expertise in understanding the needs of practicing physicians, our system is able to display the collected data on a highly visual summary page.

The Clinical Performance Summary is a graphical depiction of overall performance patterns for each physician that summarizes trends across selected clinical outcome dimensions and patient satisfaction indicators. It can be used to display comparisons within each group as well as comparisons among practices.  This tool is critical for helping physicians move towards best practices.
